New Patient Registration

When you want to register with a new GP practice, you’ll need to follow these steps to ensure that your new GP practice has all your medical history and your medical records.  If this is your first registration with a GP practice in the UK, medical records from outwith the United Kingdom will not be traced.  You can only register with a GP practice if you are staying in the area for more than 3 months. If you are not, you may still be treated, but as a Temporary Resident.

You need to complete the 2 following forms (4 pages in total):

Once completed, you can either email it to the Practice nhsh.gp55709-admin@nhs.scot or post it to either Surgery. 

To help us process your requests for repeat medications when you register with us, please can you submit:

  • A copy of your previous repeat medications slip
  • a summary of your medical records from your previous practice, or if you have a smart phone, a picture of your current medications (showing the labels) which can be emailed to our practice email address. We thank you for your co-operation with this.

On receipt of the registration form you will receive a conformation email to confirm we have received and accepted your registration.  This process can take up to 2 weeks for you to be registered. You will not be registered with any one particular doctor, but you can request to see a particular doctor if you so wish, it may just mean a longer wait for an appointment.  You may receive a phonecall from a GP/Nurse or Pharmacist to discuss any details.

Change of Details

If you change your name, address or telephone number, please notify the surgery as soon as possible in order that we can amend your details.
